In the end nobody understood Kotomine Kirei. Not his father, nor his teacher. To struggle against one's morals, to do it over and over, and to find no fitting answer is to suffer. One does not simply struggle with this propensity for a short time. You feel it every day of your life, you fight it every time you are annoyed by someone, every time you make a decision, every time you contemplate morality. Alone, with a friend, at a party, you struggle with it.
Kotomine couldn't bring himself to feel anything other than regret at losing a potential victim when his wife died. The same was for the death of his father. He would not let the third slip by.
Call it a first world problem, call it a choice towards evil, but it is the blame of this accursed human condition. That inescapable nature that we all feel, we all experience, that which binds us all.
