It gets little updates every week or two, and there are a bunch of limited time events like for Christmas, New Years, Valentines, that recent Final Fantasy collab event, etc.
Big updates are less frequent, but when they do happen they usually add in shit like new main classes (Monster Tamer is the most recent there), side story quests that expand on the backstory of characters and bosses from the main quest, the Colosseum and the Casino, the whole fucking "YOU MUST BECOME THE RULER OF TIME AND KILL THE KING OF ALL CALAMITY, ESTARK HIMSELF" thing, quest to increase current level cap by 5 levels (until very recently almost solely for the purpose of being able to stand half a chance against Estark, hah), etc.
One of the smaller updates was fairly recent actually, and added in KING HYDRA the monster that REALLY killed Ortega in DQ3. I haven't fought it yet, but I saw a video of a party of 4 people all at the level cap being completely slaughtered by it.
As far as levels go, I've only maxed out Gladiator. So that's 1 class out of 13, but I got all the others around or above level 30-50 anyway so it shouldn't take THAT long to max a few more.
Equipment, well, I've got the strongest Greatsword and Hammer in the game. Next target is the strongest one-handed sword. Armor, I've got the Battle Dress set (for levels 50 and upwards) which gives me a HUGE evasion boost, and I've almost collected the entire Great Mantle set (levels 75 and upwards), which has a 10% chance to auto-cast Oomph at the start of any battle (maximum Attack buff).
I have some pretty fun Accessories as well. The Battle Choker which is Atlas' rare drop (STR+7), a MAX enhanced Ring of Strength (originally STR+2, mine is STR+9 lol), a Power Charm (Normally STR+5, mine's STR+8), and a Black Eyepatch (5% chance to give a moderate Attack buff at the start of a battle).
I've also maxed out Gladiator, Shield, Hammer, and Greatsword skill sets, next up is one-handed sword.
As far as raw Strength goes, I've seen comparatively few people with higher. With the Greatsword equipped, my Attack is around 460. The absolute highest I've seen is about 502 which is pretty much INCREDIBLE HULK-tier.
The only bosses I haven't beat are, in order of difficulty: Belial, The Gods of Evil (Atlas, Pazuzu, and Belial, AT THE SAME TIME), Dragon Gaia (will probably be fighting this one soon since I picked up his summon card pretty recently), Baramos (last time I fought him he wiped out my party in one hit lol), King Hydra (was only added to the game like 5 days ago), Estark, and lastly (and possibly the most terrifying boss in any Dragon Quest game ever) ESTARK AFTER HE HAS LITERALLY CRAWLED THE FUCK BACK UP OUT OF HELL, BLINDED BY BLOODTHIRSTY RAGE WITH HALF OF HIS BODY ROTTING AWAY.
Estark is actually SO GODDAMN DIFFICULT that they've increased the level cap 2 or 3 times for the sole purpose of making him less overwhelmingly powerful, AND the fact that it's literally impossible with just one party so they let 2 full parties (8 people) fight him at the same time.
